Advanced primary care center to open in Minot through strategic collaboration

HealthyDakota Mutual Holdings, Crossover Health and Trinity Health to collaborate on VitellaCareSM advanced primary care center in Minot

[Minot, N.D.] HealthyDakota Mutual Holdings, Crossover Health and Trinity Health are collaborating to open VitellaCareSM, an advanced primary care center in Minot later this fall. This collaboration will help increase access to primary care providers and services in Minot and northwest North Dakota. The VitellaCare center in Minot will be managed by Crossover and located within Trinity’s Healthcare Campus and Medical District. Accessibility to these centers will be built into certain Blue Cross Blue Shield of North Dakota individual and group plans. It will offer in-person and virtual access to comprehensive healthcare services and proactive health management.

VitellaCare provides patients with a unique, holistic approach to physical, mental, and emotional well-being. VitellaCare will provide patient-focused care with a primary care provider-led team that includes nurses, doctors and mental health providers for each patient. This approach focuses on preventive, coordinated care that helps patients avoid higher cost care, such as chronic condition management and emergency interventions. Similar centers have achieved up to 15% reduction in total cost of care by reducing the need for more expensive emergency and specialty care and increasing the quality of care. Patients will have many ways to receive care from their team via video visits, messaging, phone and in-person appointments at the center.
